A Family-Centered Approach

Our adolescent program combines evidence-based medical treatment with age-appropriate counseling designed specifically for developing minds. Unlike adult programs, teen MAT includes:

The Teen Assessment Process

Initial Family Consultation:

  • Comprehensive assessment of teen’s substance use history
  • Family dynamics evaluation
  • Academic and social functioning review
  • Medical and mental health screening
  • Treatment planning with family input

Required Documentation for Teen Services:

  • Parent/guardian consent forms
  • Teen’s photo ID or birth certificate
  • Insurance information
  • School records (with permission)
  • Previous treatment history (if applicable)
  • Medical records related to substance use
