Assertive Community Treatment

ACT is an intensive, community-based service for clients with a history of frequent psychiatric hospitalizations.

Chemical Dependency Programs

Metrocare’s chemical dependency programs are designed to provide effective treatment for adults with substance use concerns in a therapeutic outpatient environment.

Homeless & Housing Services

Metrocare provides an array of housing services, including homeless outreach, homeless clinics, short-term transitional housing, rehabilitative support services, housing placement assistance, and support for adults 18 and older who are mentally ill.

Military Veterans & Family Members

Metrocare Veteran Services provides mental health, case management and traditional supportive services to veterans and their families through The Steven A. Cohen Military Family Clinic.

Projects for Assistance in Transition from Homelessness

PATH provides services to people who have a severe mental illness and/or co-occurring substance use disorder as well as experiencing homelessness or are at risk of homelessness. Outreach Workers look for individuals on the street, make contact with them and attempt to establish a trusting relationship that will enable the person to come to the COC clinic and utilize the mental health services available to the homeless.

Pharmacy Services

Metrocare offers the convenience of onsite pharmacies at most of the Metrocare centers where clients can fill necessary prescriptions.

Primary Health Care

Primary Care at Metrocare utilizes an integrated care model to offer adults and children primary care and behavioral health care at the same location. We are pleased to offer family medicine and pediatrics at several of our selected sites.

Rapid Assessment & Prevention

RAP Team (Rapid Assessment and Prevention) is a mobile service, allowing caseworkers to meet with you at your home or in the community. These flexible, hardworking caseworkers can provide coordination to clinic appointments or even help you to link to community supports for food or clothing. The purpose of the RAP team is to help clients to meet their own basic needs, remain psychologically healthy, and become or remain independent.
