Metrocare Services Open Records Request

Any member of the public has the right to access, to view, or receive copies of certain information about Metrocare Services (“Metrocare”) under the Texas Public Information Act (the “Act”).  The type of information subject to disclosure under the Act is described in detailed in at Chapter 552 of the Texas Government Code.  Metrocare will ensure that any open records requests will be answered in a timely manner and the information or records requested for inspection or receipt will be provided promptly or at an estimated, reasonable time frame in which the information or records will be available for inspection or can be reproduced to provide for receipt.  

*Please note that any requests for medical records and/or personal health information relating any treatment or payments for healthcare services or other clinical, therapeutic, or support services provided by Metrocare Services will need to be directed to the Medical Records Department by mail at: Metrocare Services, Attn: Medical Records Department, 1330 River Bend Drive, Dallas, TX 75247, or by telephone at 214-743-1222.  Requests for medical records and/or personal health information requires the proper consents and authorizations for release of information from designated persons as required by law. 

Rights and Responsibilities of the Requestor

  • Receive prompt access of information that is not protected or restricted by law
 
  • Be treated equally with all other requestors
 
  • Receive, in advance, a written statement of estimated charges when charges will exceed $40 before any work is started to complete the request
 
  • Modify request in response to the written statement of estimated charges
  • Choose whether to inspect or receive copies of requested information, or both
 
  • Request for a fee reduction or waiver if the information is a matter of public interest
 
  • Be notified if Metrocare seeks a ruling from the Texas Office of Attorney General for an exception to the Act
 
  • File a complaint for excessive charges or for information not provided as requested
  • Submit requests for open records in writing wtih clear and specific description of the information requested in order for Metrocare to accurately identify and locate the requested information
 
  • Pay all reasonable fees in a timely manner
 
  • Cooperate with Metrocare to clarify the type and amount of information requested
 
  • Keep appointments to inspect records or pick up copies of information requested
