Family Preservation Program

The Family Preservation Program (FPP) is an intensive, community based service whose purpose is to prevent out of home placements into hospitals, juvenile justice facilities, or residential treatment programs. A Qualified Mental Health Professional (QMHP) works closely with the family to identify and build upon the family members’ strengths, and to reduce risk factors for out of home placement through crisis management, safety planning, behavior modification, and skills training. Our QMHP will provide services at a minimum of eight hours per month for a maximum of three  months.


  • Ages 3 -17
  • Are at imminent risk for out of home placement
  • Have been recently discharged from psychiatric hospital, juvenile justice facility, or residential treatment program
