RAP Team (Rapid Assessment and Prevention)

RAP Team (Rapid Assessment and Prevention) is a mobile service, allowing caseworkers to meet with you at your home or in the community. These flexible, hardworking caseworkers can provide coordination to clinic appointments or even help you to link to community supports for food or clothing. The purpose of the RAP team is to help clients to meet their own basic needs, remain psychologically healthy, and become or remain independent.

What does RAP do?

Rehab Services

The RAP Team works with each consumer on his/her specific needs. Some consumers need assistance in riding the bus, keeping their house clean, making and keeping appointments, budgeting, managing their illness, or grocery shopping. Some needs skills training with managing depression, anger management, stress and anxiety management, etc. The RAP Team is available to help where needed.

Case Management Services

The RAP Team assists consumers in linking to community services to meet their basic needs, such as housing, employment, clothing, food stamps, medical treatment/insurance, SSA income, ID/birth certificates, etc.

Skills Training Services

Well-trained and experienced clinicians from a variety of backgrounds are available to provide services including skills training in areas such as interpersonal issues, substance abuse, depression and anxiety, and relationship issues. Group therapy is also available to focus on topics such as life skills, symptom management, and anger management.
