Payment options

Metrocare accepts multiple forms of payment including Medicaid, Medicare, some private insurance and self-pay. Initial appointments include a financial assessment to determine what payer source best fits the needs of the individual receiving services. 

Medical or treatment records request

Requests for copies of any medical, treatment, or financial records or records of other services provided by Metrocare must be sent to Metrocare Services Release of Information (“ROI”) Department. Please visit the medical or treatment records request page for instructions on how to request records for yourself, your child or another person.
