PURPOSE OF JOB:
The Individual Service Coordinator (ISC) will work as member of the Service Coordination Unit and is responsible for monitoring and managing the GRA functions according to established policies and procedures, authorization of MR/RC Assessments, ICAP Assessment, Person Directed Plan, authorization of Individual Plan of Care, Provider Choice, field based monitoring of services, and Consumer Satisfaction.

EDUCATION AND EXPERIENCE:
Bachelor’s degree from an accredited college or university with a major in the human services or related field with one (1) year of full-time experience working in the field of mental retardation or other developmental disabilities.  Bilingual preferred.  Must possess a valid drivers’ license.

Responsible for the clinical supervision and training for Customer Services Coordinators (CSC), as well as monitoring and managing the HCS functions according to established policies and procedures.  The Team Leader is responsible for understanding and assuring that the CSC’s fulfill and perform their responsibilities according to established policies and procedures.

EDUCATION AND EXPERIENCE:
Masters degree in Human Services Field. Bachelors degree with equivalent work experience may substitute.  Experience working with developmental disabilities required.
