Job Title: Customer Service Coordinator

Salary: $32,500 (Bilingual in Spanish -- $35,000)

PURPOSE OF JOB:

The Individual Service Coordinator (ISC) will work as member of the Service Coordination Unit and responsible for monitoring and managing the GRA functions according to established policies and procedures, authorization of MR/RC Assessments, ICAP Assessment, Person Directed Plan, authorization of Individual Plan of Care, Provider Choice, field based monitoring of services, and Consumer Satisfaction.

EDUCATION AND EXPERIENCE:

Bachelor’s degree from an accredited college or university with a major in the human services or related field with one (1) year of full-time experience working in the field of mental retardation or other developmental disabilities.  Bilingual Required.  Must possess a valid drivers’ license.
