To ensure that your first visit is as convenient and efficient as possible, we are pleased to offer our registration forms and information online. Please print and complete the below forms and bring to your initial intake appointment. Other forms may be required to be completed in person.
Payment Options
Metrocare accepts multiple forms of payment including Medicaid, Medicare, some private insurance and self-pay. Initial appointments include a financial assessment to determine what payer source best fits the needs of the individual receiving services.
